Immunization is one of the most important accomplishments in public health that has, over the past 50 years, led to the elimination, containment, and control of diseases that were once very common in Canada. Vaccines help the immune system recognize and fight bacteria and viruses that cause diseases.
West Park Manor PCH will be offering the Influenza Vaccine and Pneumococcal Vaccine to residents in mid-late October 2023. ONLY eligible Residents will receive a Pneumococcal vaccine or booster.
COVID-19 vaccinations will be offered mid-November when the COVID vaccine becomes available. We will ask for your consent for the COVID vaccine at a later date.
Consent forms for the Influenza &/or the Pneumococcal vaccines are available at the 1st & 2nd floor nursing stations. When you are visiting your resident please sign the consent forms in person at the 1st or 2nd floor nursing stations. The Nursing staff will inform you if your resident is eligible for the Pneumococcal vaccine. Please inform us if you are declining any these vaccines.
You may also telephone West Park Manor Nursing Station to give verbal consent.

We are not accepting consent forms by email due to the confidential information that is on the consent form.
Please read the attached factsheets prior to signing the consent forms and let us know if you have any questions or concerns regarding the vaccines.
Consent forms should be completed by Sunday, October 8, 2023. You will receive a telephone call for verbal consent if the consent forms are not completed by this time.
